Sports Forms and Physicals
Sports physicals will be available for FREE Tuesday, August 11 and Wednesday, August 12 from 9:00 - 2:30 each day at SVMS.
*** Please get new sports physicals done! They must be renewed at least once every 15 months. If your child has seen their primary care physician recently, please call their office to get a sports physical clearance form from them!
